Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice.  This courtesy, on your part, will make it possible to give your appointment to another patient.

Health Insurance Plans

Our office participates with most health insurance plans which cover certain procedures.  Covered procedures will vary with each insurance company -- our office staff will assist you in determining eligibility.  Please contact our office at (650) 692-0182 to find out if we accept your insurance.  Patients are responsible for deductibles, co-payments and remaining balances.  Patients also have the option of paying office and surgical fees in advance and then receiving their insurance reimbursement check directly from the insurance company.

Financial Policy

Co-payments and co-insurances are due at the time services are rendered.  Forms of payment accepted by the office are check, cash, insurance assignment and the following credit cards:

Emergencies

If you have a medical emergency, our office provides an on-call service, 24 hours-a-day, 7 days-a-week.  Call (650) 692-0182 and one of our doctors will be contacted immediately.
