If you need to have your medical records sent to or transferred to another offfice, physician, or insurance company please complete this form and either mail or fax into our office.

Our office participates with most health insurance plans which cover certain procedures. Covered procedures will vary with each insurance company — our office staff will assist you in determining eligibility. Please contact our office at (650) 692-0182 to find out if we accept your insurance. Patients are responsible for deductibles, co-payments and remaining balances
